REGISTRATION

At the time you register, your form will be numbered. Each form will be processed in number order and on a first-come, first-served basis. In the event a program fills, you will be placed on the waiting list for that class and notified.

1. All in-person registration for the programs will be held at the Palos Heights Recreation Center.
2. Registration will be on a first-come, first-served basis with in-person registration taking priority.
3. Proof of residency may be required at the time of registration.
4. Phone registration will not be accepted unless otherwise specified.
5. Mail-in or drop-off registrations received prior to registration dates will be processed at 5:00pm on the first day of sign-up for residents and non-residents. When there is a class limit, mail-in or drop-off registration does not guarantee a place in any specific activity.
6. When a class limit is reached, a waiting list will be started. If enough interest is shown and the facility and instructor are available, another class will be started. The Recreation Department reserves the right to cancel any program or trip due to insufficient registration.
7. For registration purposes, the Recreation Center opens at 6:30am. Numbers will be passed out at 8:00am.
8. Each registrant is allowed to sign up an additional family at the time of registration. This policy may be suspended for specific trips and shows. If so, it will be noted in the program description.
9. There are no substitutions on trips. A transfer must be done prior to the trip.

REFUND POLICY

There will be a service charge of $3.00 on all refunds except when the Recreation Department cancels the program, and then a full refund will be given. Any other refund given is less class time elapsed. There will be NO REFUNDS issued after the second class, no exceptions. Please allow 3-4 weeks for your refund. There will be no refunds on any trip/tickets unless a replacement is found. Due to the preparation and planning required for special events, there will be no refunds issued within one week of the event date.

SENIOR CITIZEN DISCOUNT

All individuals age 60 and older may receive a 25% discount on any adult activity excluding trips and certain special events. Proof of age (driver’s license, birth certificate, etc.) will be required upon registration to receive discount.

TRANSFER POLICY

There will be a service charge of $3.00 on all transfers between classes. Transfers due to closed or cancelled classes will be done at no charge. No transfer is allowed after the second week of class.

RETURNED CHECKS

There will be a $20.00 fee assessed for a returned or NSF check.

FINANCIAL ASSISTANCE POLICY

It is the policy of the Palos Heights Recreation Dept. that no resident shall be denied participation in a program due to the inability to pay. If any resident is unable to afford a Recreation Dept. program, please contact Mark Purcell, Director of Parks & Recreation, at 708-361-1807 to discuss the scholarship program. All applications are confidential.

AGE REQUIREMENT

Unless otherwise stated, participants must be the age required in the program description by the first day of class. All of our programs and classes are designed for participants of the age indicated. Thank you for your honesty and cooperation.

PHOTO POLICY

Photos are periodically taken of participants in a class, during special events, or at the Recreation Center. Please be aware that these photos are for the Recreation Department’s use only and may be used in the Recreation Department’s brochures, calendar, pamphlets or flyers.

WAIVER REQUIRED

For insurance purposes, Recreation Department programs and activities require a signed waiver. Please sign the waiver section on the registration form. Patrons WILL NOT be able to participate in classes or programs if the waiver is not signed. Refunds cannot be given for failure to sign the waiver.

NON-RESIDENT FEE

The non-resident fee for programs will be $20 or 50% more than the resident rate, whichever is less. The non-resident fee is charged to offset the costs charged to residents in the form of property taxes.

CLASS LOCATIONS

All classes are at the Palos Heights Recreation Center, 6601 W. 127th Street, unless otherwise noted.

REGISTRATION BY/CANCELLATION DEADLINE

This date is when a class will be cancelled if enrollment is insufficient. Registration will be accepted after this date if the class has not been cancelled and space is available.

CODE OF CONDUCT

Participants will:

1. Show respect to all participants & staff, take direction from staff.
2. Refrain from using abusive or foul language.
3. Refrain from causing bodily harm to self, other participants or staff.
